Facing an injury or illness can be a difficult time for patients and families. Often, there are unforeseen circumstances that arise that make the situation more stressful. Confronting tasks such as lining up care at home and managing medical and prescription costs can be overwhelming when struggling with illness or recovery. The Finley Hospital offers social work services to help meet these and other social and emotional needs of patients and their loved ones in both the hospital and outpatient settings.

Through interactions with the entire healthcare team, social workers identify patients with potential needs and complete a comprehensive assessment that determines areas of current and future concerns. Social workers provide information on available community resources to help patients and families maintain the safest, most independent lifestyle possible. Interventions may vary from lining up transportation, assisting with financial concerns, arranging for care at home or in a nursing home, and helping patients and families cope with life changes related to illness.

Case Management

A hospital stay can be complex and stressful for patients and their families. RN Casemangers are available to help. RN Casemanagers work with patients, families and the healthcare team members to coordinate resources to ensure that necessary services are provided at the most appropriate level of care, and that there is a smooth progression for the patient thru the continuum of care. They help to overcome or eliminate obstacles that may prevent our patients from having optimal outcomes. They communicate between patients, families, healthcare team members and payors. Casemanagers are advocates for patients and their families as they provide support with life transitions. They are available to provide education regarding availabler health care resources. They will work with the entire healthcare team to help you understand your treatment plan and help you to feel ready for discarge from the hospital.
